Neck Lift

Loose skin, excess fullness, muscle laxity, and changes to the deeper structures of the neck can gradually blur the distinction between the face and neck. These concerns may be related to aging, genetics, weight changes, or a combination of factors - and they often cannot be fully corrected through diet, exercise, or nonsurgical treatment alone.
Mays Plastic Surgery offers two surgical approaches to neck rejuvenation: the traditional neck lift performed by Dr. Chet Mays and submentoplasty performed by Dr. Jeffrey Gross*. Although both procedures can create a smoother neck and more defined jawline, they address different anatomical concerns and use different surgical techniques.

What Is a Neck Lift?
A neck lift is a surgical procedure that improves contour beneath the chin and along the upper neck.
Unlike treatments that only remove fat, neck lifts allow Dr. Gross to address multiple factors that contribute to fullness and poor neck definition, including:
- Excess fat beneath the chin
- Loose or separated neck muscles (platysma)
- Loss of jawline definition
- Early signs of neck aging
- Poor neck-to-chin angle
- Ptotic submandibular glands
Through a small incision hidden beneath the chin, underlying structures can be refined and repositioned to create a smoother, more defined contour.
The result is a stronger jawline, a more youthful neck profile, and improved facial balance.
Traditional Neck Lift with Dr. Chet Mays
A traditional neck lift is generally intended for patients with more noticeable skin laxity, muscle banding, or age-related changes extending across the neck and jawline.
Dr. Mays customizes the procedure to address the patient's individual concerns, which may include excess skin, localized fat, and laxity of the platysma muscles. By improving these areas together, a traditional neck lift can create a smoother neck contour and restore clearer definition along the jawline.
A traditional neck lift may be appropriate for patients experiencing:
- Loose or hanging neck skin
- Visible neck bands
- Fullness beneath the chin
- Loss of jawline definition
- Moderate to advanced neck aging
- A "turkey neck" appearance
The procedure may be performed independently or combined with a facelift when aging affects both the lower face and neck.
Submentoplasty with Dr. Jeffrey Gross
Submentoplasty focuses primarily on the area beneath the chin and the deeper structures that influence the neck-to-chin angle. Dr. Gross performs the procedure through a carefully concealed incision beneath the chin, allowing him to address concerns that may not be corrected through fat removal alone.
Depending on the patient's anatomy, submentoplasty may include removing excess fat, tightening or repositioning the platysma muscles, and refining deeper structures of the neck. For appropriately selected patients, Dr. Gross may also remove the ptotic submandibular glands (salivary glands) that contribute to persistent fullness beneath the jawline.
Submentoplasty may be appropriate for patients experiencing:
- Fullness beneath the chin
- A persistent double chin
- Poor separation between the face and neck
- A weak or poorly defined neck-to-chin angle
- Platysma muscle laxity
- Ptotic submandibular glands (salivary glands)
- Limited excess neck skin
- Because submentoplasty primarily addresses the central neck and underlying anatomy, it is not a substitute for a traditional neck lift in every patient.
Mays Plastic Surgery & Med Spa Reviews
Dr Mays is a true professional. He and his team do great work!
Dr Mays and his staff are truly one of a kind and flat out AMAZING! The minute I had my first consult with him I knew I didn't need to look any further. I am 8 months post-op from lipo, breast reduction and tummy tuck. Unfortunately, I had a little setback from a large seroma several months ago that caused some elasticity problems. Dr Mays was confident my revision two weeks ago would yield the results I wanted. He was not wrong! I could not be more happy with my new look. Thank you for everything!
I love the ways he treated us and the results are amazing - Best plastic surgeon in Louisville KY! Indeed! Dr mays is a combination of knowledge and kindness. He really cares about what he does and the patient in front of him! I love the ways he treated us and the results are amazing !!!!

What Concerns Can a Neck Lift Improve?
Depending on the procedure selected, surgical neck rejuvenation may improve:
- Excess fat or fullness beneath the chin
- Loose or separated neck muscles
- Sagging neck skin
- Visible neck bands
- A poorly defined jawline
- A weak neck-to-chin angle
- Ptotic submandibular glands (salivary glands)
- An indistinct transition between the face and neck
The goal is not to create an overly tight or artificially sculpted appearance. Both Dr. Mays and Dr. Gross tailor treatment to restore cleaner, more youthful contours while maintaining balance with the patient's natural facial features.
Which Procedure Is Right for You?
The most appropriate approach depends on where your concerns originate. Some patients primarily need excess skin removed, while others have fullness related to fat, muscle laxity, or deeper neck structures. In some cases, multiple factors contribute to the appearance of th eneck.
During your consultation, Dr. Mays or Dr. Gross will:
- Evaluate your neck, chin, and jawline anatomy
- Assess skin laxity, fat distribution, and muscle position
- Determine whether deeper structures contribute to fullness
- Discuss your concerns and aesthetic goals
- Explain the benefits and limitations of your surgical options
- Develop a personalized treatment plan
Patients who initially believe they need submental liposuction may learn that skin laxity, muscle position, or ptotic glands also contribute to their concerns. A comprehensive evaluation helps determine whether liposuction, submetoplasty, a traditional neck lift, or another approach is most likely to provide a balanced result.

NeckLift FAQs
Is a neck lift the same as neck liposuction?
Not exactly. Liposuction removes excess fat, while neck lifts allow the surgeon to address fat, muscle, laxity, and deeper neck structures that contribute to poor definition.
Will I have a visible scar?
The incision is typically small and hidden beneath the chin, making it difficult to notice once healed.
How long do results last?
Results are long-lasting, particularly when a stable weight is maintained.
Can diet and exercise fix a double chin?
For some individuals, yes. However, many patients have genetic fat distribution patterns or muscle laxity that cannot be fully corrected through weight loss alone.
