• Earn points for services and procedures rendered in-office, product purchases, and surgeon's fees.
• You will earn 1 Ava Reward point for each dollar spent on qualified purchases.

REDEEM YOUR POINTS

• At checkout, we'll let you know how many Ava Rewards you have available to redeem. You may choose from four tiers of popular products and services.

• Points never expire. Redeem your points when you want, or save them to redeem larger rewards.

• Rewards are refreshed quarterly so you will always have something exciting to look forward to!

Rules and Restrictions

• Branded swag items and skin care items are based on availability.
• Points cannot be earned on sales tax, facility fees, or anesthesia fees.
• Points can only be redeemed in one transaction per month.
• Neurotoxin (You may choose up to 2 areas. Up 40 units of BOTOX or 100 units of Dysport)
• Filler (one syringe)
• You may not transfer points to another patient.
• The patient who earned the points can be the only one to redeem them.
